A near-total breakdown in policing protocols hindered the response to the 2022 school shooting in Uvalde, Texas, that left 21 people dead — and the refusal to rapidly confront the killer needlessly cost lives, the Justice Department concluded on Thursday after a nearly two-year investigation. The department blamed “cascading failures of leadership, decision-making, tactics, policy and training” for the delayed and passive law enforcement response that allowed an 18-year-old gunman with a semiautomatic rifle to remain inside a pair of connected fourth grade classrooms at Robb Elementary School for 77 minutes before he was confronted and killed. BOYNTON BEACH, Fla. (AP) — A Florida man already charged with murder has been indicted for the killing of a Lyft driver whose car he allegedly stole in an attempt to escape, authorities announced Wednesday. An Okeechobee County grand jury this week indicted Mathew Flores with first-degree murder and armed robbery for the Jan. 30 slaying of 74-year-old Gary Levin. Flores is being held without bail on both murder charges. Levin's body was found five days after his slaying by investigators who retraced his ride with Flores, Lawson said. CNN —An underwater search team that specializes in missing persons cold cases discovered approximately 30 cars submerged in a South Florida lake over the weekend, police said. “As of this time, it is unknown if these vehicles are linked to criminal activity nor how long the vehicles have been submerged in the lake,” Doral police said. Authorities were investigating Tuesday after divers recovered dozens of vehicles from a lake in Doral, Florida. More than 40 cold cases on are their radar in both Miami-Dade and Broward counties, Bishop said. Miami-Dade police told CNN their divers were working Tuesday to recover vehicles from the lake with assistance from Miami-Dade Fire Rescue.
